Winner selection and survey results 

The drawing will be held at St. Jude Eye Care “the Practice” on the 1st of every month. Winner need not be present to win. The winner will be determined by a random drawing of collected surveys. The winner will be notified via contact information supplied on the survey. In the event of extenuating circumstances, if the drawing cannot be conducted as planned on 1st of the month, the drawing will be completed on the next available business day.

Use of your personal information

We are committed to protecting your privacy. We will never use your name without your consent.  Satisfaction ratings and feedback may be aggregated and published anonymously on the Practice website. If indicated by you, this Practice may contact you to conduct research about your opinion of current services or of potential new services that may be offered.  If you indicated, you may receive newsletters about the Practice and services that may be offered.  This Practice does not sell, rent or lease its customer lists to third parties.
